Request a new Facilities Management (FM) Asset Project File

This form is for requesting FM Asset Project Files for Facilities Management only, and is for the use of the Project Officer, Project Support Officer, or Administrative Assistant (Reception).

To ensure files are created as quickly as possible, follow these directions.

  • Fill out all mandatory fields (marked with an *).
  • For any project that is not asset-specific, indicate this by entering N/A in the Building Name and Asset No fields.
  • Include a description of the project in full without shortened forms of words.
  • If an external Project Manager is being used, indicate the name and company in the Project Manager field, and the name of the FM contact person in the Project Leader field.

A TRIM reference will be sent to the Requesting Officer to distribute to the relevant staff and attach the files to an “FM Projects” User Label.
